Information and Documents we Need

We've tried to make this part of your trip as quick and painless as possible with two easy online forms:

1.Health & LIfestyle Form - This must be completed by or for each Guest.
Among other things this form is used for menu planning. Menu planning and food shopping are done well in advance.

Each Guest will need to log in with their Guest ID (from your confirmation or status letter) and email address that we have on file for that guest.

** Before completing the form, you should have on hand your insurance information and contact details for 2 emergency contacts.

2. Arrival, Accommodation & Rentals Form - This only needs to be done once for the group that has registered together.. Save 10% on gear rentals if you complete this form before making your final payment.

**Before you submit this form please have ready you complete flight information, hotel information and your decision on your rental equipment.

In addition to the Online Forms, we need the following two documents signed and returned via Mail, or by scan and email.

3. Contract
This form clearly spells out what is provided by Exposure and what is expected of our guests.Please return this completed form as soon as possible. A copy of this form is included in your trip packet that you can download below.

4. Acknowledgement of Risk & Release (Waiver)
This legal document is intended as a warning that not everything we're going to do is risk-free and to advise you of some of the risks you might be up against.
It is important that you understand, and accept, the risks involved with participatioin in this program. We must receive this form in advance of your trip in order for you to participate. We cannot accept it the same day your trip starts. A copy of this form is included in your trip packet that you can download below.

If it is less than one month before your trip start date, you can either scan and email the forms back to us at adventures@exposurealaska.net or fax them to 206-984-0174.

If it is a month or more before your trip start date you may also mail them to:

Custom Family Trips
The Waiver, Health & Lifestyle Form should be completed for all participants, including children.

The Contract need only be signed by the Leader of the Family Group.

5. Your Final Payment -- Is due by check mailed to the address above, ideally with your paper forms. If you really want to pay with MasterCard or Visa, we can accept that if you agree to pay the full balance which includes the 3% merchant fee. The payment by credit card balance can be found on your latest "Status Report" email or contact us for an updated balance. We require a signed Authorization Form for those larger payments. More about the Acknowledgement of Risk & Release

An individual Acknowledgement of Risk must be signed by each member of the group, including minors.

No actvity is completely without risk. Our programs take place in the wilderness, in the real Alaska. We do our best to minimize risk while providing the opportunity for a challenging and rewarding experience.

PLEASE READ this document carefully NOW. Your acknowledgement and signature on this document as presented will be required for participation. Click above to download this form in the Exposure Trip Packet.

Please do not hesitate to contact us with any questions regarding this document or the risks associated with your participation on an Exposure trip.

During the trip you may also be asked to sign an additional waiver for any sub-contractors that we may use; whitewater rafting for example.
