The following 4 forms are needed for your participation:

1. Health & Lifestyle Form
This questionnaire provides us with basic health, fitness and experience information about our guests. It's a little more involved than some programs but we think that it is a valuable tool to adjust our trips to suit the team. Please return this completed form as soon as possible. -- This is best done here on-line at:

2. Contract
This form clearly spells out what is provided by Exposure and what is expected of our guests.Please return this completed form as soon as possible. A copy of this form is included in your trip packet that you can download below.

3. Arrival Details
This short form gives us those final few details we need to find you in Anchorage. Please return this completed form as soon as you know your details. Please do NOT hold up the other forms while you finalize your travel details. Arrival Details

4. Acknowledgement of Risk & Release
This legal document is intended as a warning that not everything we're going to do is risk-free and to advise you of some of the risks you might be up against.
It is important that you understand, and accept, the risks involved with participatioin in this program. A copy of this form is included in your trip packet that you can download below.

Remember:

-- Complete and return the forms as soon as possible.

-- Send in your final payment.

-- Book your flights and Anchorage accommodation.

-- Get your equipment together.

-- Don't hesitate to contact us with any questions.

Custom Family Trips
The Health & Lifestyle Form should be completed for all participants, including children.

Arrival Details may be completed once for the group if traveling together.

The Contract may be signed by the leader of the group.

A seperate Acknowledgement of Risk must be signed by each member of the group, including minors.

No actvity is completely without risk. Our programs take place in the wilderness, in the real Alaska. We do our best to minimize risk while providing the opportunity for a challenging and rewarding experience.

Please do not hesitate to contact us with any questions regarding this document or the risks associated with your participation on an Exposure trip.

During the trip you may also be asked to sign an additional waiver for any sub-contractors that we may use; whitewater rafting for example.
